Job Summary

The Case Management Nurse will provide expert support in managing patient care plans and improving health outcomes.

Key Responsibilities:

* Oversee and coordinate comprehensive care plans for beneficiaries.

* Develop case management protocols that align with TRICARE policies.

* Collaborate with healthcare providers to address patient needs effectively.

Qualifications:

* Bachelor's degree in Nursing (required); Master's in Health Services or related field preferred.

* 2 years of DoD Program Management experience within the last 5 years (preferred).

* In-depth knowledge of MHS care coordination processes.
